IMMEDIATE HIRE! Fast-growing 4.5 STAR rated home health company has immediate opening for skilled visiting RN's in Cook, DuPage, Will, Kane, and Lake Counties to be a part of our experienced clinical team.
QUALIFICATIONS of Registered Nurse :
· RN nurse license required
· Must have Home Health SOC experience
· Current CPR Certification
· Valid Driver's license
· Valid Car Insurance
· Familiar with Axxess EMR Software
· Excellent communication and time-management skills
RESPONSIBILITIES of Registered Nurse:
- Completes 3-4 Start of Cares daily and generate the Plan of Care.
- Ability to drive to Cook, DuPage, Will, Kane and Lake Counties.
- Provides friendly and professional service in person, over the phone and through written communications.
- Provides infusion and other skilled nursing care to patients including, but not limited to; patient assessments, patient/caregiver teaching, development and revision of the plan of care, implementation of physician orders, monitoring patient response, response to emergency patient needs and psychosocial support.
- Safely and accurately administers medications, without errors, including oral, IM, SQ, and IV products, when necessary.
- Dress or redress open wounds and assess progress of healing.
- Completes all required patient care documentation and reports accurately.
- Submits all required patient care documentation and reports in a timely manner.
- Communicates to Agency Supervisor, physicians and other health care providers, as appropriate.
- Takes on call responsibility as assigned to provide 24-hour availability and provides these services in accordance with established protocols, when necessary.
- Takes professional responsibility for self-protection during required work in potentially hazardous situations (e.g. handling of blood and body fluids).
- Uses technology to thoroughly document all patient care activities and interventions according to organizational policy and procedure.
- Accurately performs necessary nursing responsibilities including, but not limited to obtaining lab specimens, performing vital signs, and administering oxygen therapy.
- Demonstrates appropriate storage and handling of drug products as well as human specimens.
- Counsels/advises patients regarding symptom/side effect management during therapy and within the scope of practice.
- Teaches patients and family members regarding the treatment care plan and what to expect and thoroughly documents the teaching and response to teaching.
- Collects and records all patient data as required per the care plan and/or other protocol and analyzes patient trends and outcomes.
- Maintains a current knowledge of all equipment utilized and performs calibration, and quality assurance of all equipment.
- Actively participates with organizational quality and performance improvement efforts.
- Supports the account executives and clinical liaisons by providing educational programs or visits to referrals sources, as requested.
- Positions available for Cook, DuPage, Will, Kane, and Lake Counties.
- Per Diem: $50 - $110.
