Salary Range: $76,000 – $79,000 DOE

Deliver skilled, compassionate nursing care in patients’ homes and facilities.
Conduct comprehensive assessments and develop individualized care plans that honor patient wishes.
Provide education, emotional support, and guidance to families and caregivers.
Coordinate care with the interdisciplinary team to ensure seamless communication and holistic support.
Administer medications, treatments, and symptom management with excellence and grace.
Participate in on-call rotations to ensure patients always receive timely, dignified care.
Active RN license (required).
Experience in hospice, palliative, geriatric, or home health care (preferred).
A calm presence, strong clinical judgment, and a compassionate heart.
Ability to work independently while collaborating with a dedicated care team.
Excellent communication, documentation, and organizational skills.
