Ijn Health Systems PA is a Skilled Home Health Agency in Chadds Ford , PA is looking for License Practical Nurse (LPN) to join our strong and compassionate caregivers . Our ideal candidate is a self-starter, reliable, motivated, and engaged.

License Practical Nurse (LPN/ LVN) – Home Care

Responsibilities:

• Assess, plan, implement and evaluate patient care.
• Administration of medications and documentation
• Health promotion, teaching, and training of family members
• Wound care 
• Patient assessments, coordination of care
• Monitor and report changes in patient symptoms or behavior

Qualifications:

• L.P.N, LVN or state specific licensed nurse credential
• Must be at least 18 years of age
• Previous Home Care experience preferred and desire to serve and care for seniors.
• Must be licensed and in good standing (LPN) in the state of Delaware
• Minimum of six (6) months experience working with seniors in assisted living, home health, independent living, hospital or long term care environment and desire to serve and care for seniors
• Ability to react and remain calm in difficult situations
• Ability to demonstrates good judgment, decision making skills and problem solving .
• Possess written and verbal skills for effective communication and level of understanding
• Drivers License and good access to transportation
• CPR
