HOME HEALTH / CARE TRANSITION NURSE (RN)

The  Care Transition Nurse (RN) in Home Health will collaborate with physicians, hospitals, nursing homes, and home care personnel in the process of providing coordinated infusion based services for patients referred to the organization.  This individual will also increase awareness among key contacts at referral sources regarding infusion care and helps coordinate services for appropriate individual patients.  The Care Transition Nurse assists patients in making a seamless transition to infusion services.

Key Job Responsibilities

 

• Supports strategic goals with high standards of customer service
• Follows the policies and procedures of the organization
• Adheres to the corporate compliance program, including confidentiality of HIPAA protected health information.  Maintains confidentiality of patient information.
• Works independently with limited direct supervision using the guidelines of the Nurse Practice Act, Organization’s and department policy and procedures, and professional judgment.
• Observes and reports trends, changes and new opportunities and identifies ways to educate and advance providing infusion as an optimal solution as appropriate.
• Develops professional working relationships with health care providers and facilities; provides value added counsel, teaching and resourceful problem solving.
• May assist in the education of providers and personnel on home care regulations and the variety of services available as well as industry trends.
• May participate in educating and in-servicing new providers and personnel on home infusion and the importance of communication of this information to their patients.
• May act as a general home care resource for patients choosing the organization’s infusion services.
• Provides infusion education to patients, caregivers and others as requested by referral sources.
• Promotes effective working relationships with nursing personnel, management team and healthcare providers.

Licensing, Education & Experience:

• Current RN licensure in state of Indiana;  Home Health experience a plus

• Current CPR certification required

• Current Drivers License, vehicle insurance, and access to a dependable vehicle
