Due to critical service need of the communities we serve, we are offering a $250 sign-on bonus.* We are eager to continue to provide quality and compassionate care in our service areas and need you to make this happen!
PRN/as needed position; availability required for evening, overnight and/or weekends hours.
The Community Living Instructor provides resident care and supervision to assigned residents and maintains resident environment and safety.
Responsibilities Include:
* Assist residents with Activities of Daily Living (ADL's) to include but not limited to: bathing, showering, dressing and grooming, shaving, nail care, hair care, eating, making bed, laundry; document care given daily on Personal Care Documentation Form for residents as assigned
Also, assist serving meals to residents
* Provide treatment interventions to residents in alignment with their individualized treatment plan
* Hourly resident checks as assigned
* Maintain resident Smoke Schedule and supervise residents for safety. Including stocking cigarettes
* Encourage residents to participate in activities, day program, socialization, work schedules, and regular exercise
* Monitor and document resident behavior (ICPR progress notes – three notes per shift)
* Assist client in meeting their general health needs, i.e., transporting to scheduled appointments for eye exam, routine physical exams, dental exams, yearly OB/GYN exams or follow-up testing as ordered by physician—this is indicated on the daily assignment sheet
* Administer medications as ordered by physician:
* Narcotic Count is done q shift
* Transcribing MD orders
* Ordering and reordering necessary medication/ supplies
* Signing in totes of medications received
* Documenting medications administered on MAR
* Review, Understand, and Adhere to the S26 Keaton ALF Manual.
* Review and Complete all necessary documentation as outlined in S26 Manual, including but not limited to: the Monthly census report; Daily Assignments; MARs and Control sheets; etc.
WORKING CONDITIONS & EXPOSURE RISK
Regular exposure to biohazard and chemical waste materials
EDUCATION
High School/GED required
WORK EXPERIENCE
* At least one (1) year of work experience or self-employment required
* A valid unrestricted driver's license at the time of hire with the ability to obtain and maintain a class E MO driver's license by the end of 60-day introductory period required
